By offering cutting-edge treatments, innovative research, and a patient-centered approach, we are redefining what’s possible in the fight against cancer and blood disorders.If you want to be part of a growing organization committed to healing, hope, and advanced care, join us and help make a meaningful impact!Job Description:LPN Clinical Care Team CoordinatorLocation: Bohemia, NYHours: Monday–Friday (8.5hr shifts)Organization: New York Cancer & Blood Specialists (NYCBS)In This Role, You Will:Monitor and manage patient communications, ensuring timely responses and appropriate action on messages related to patient care coordinationPatient outreach and education and clinical questions; within the LPNs scope of practiceGuide patients through their care journey by facilitating appointments, follow-ups, and ensuring patients have access to necessary servicesAssist centralized FMLA team with completion of forms requiring provider signatureProcess and coordinate prescription refill requests in collaboration with healthcare providers, ensuring accuracy and timely deliveryUtilize Prescription Monitoring System (PMP) for refill requests of controlled substances, ensuring proper follow-up is scheduledLiaise with the revenue cycle team to provide patients with updates on authorizations and referralsArrange Peer to Peer conversations with providers to ensure authorization of testing and medications are completed in a timely mannerWork closely with physicians, nurses, and other healthcare professionals to develop andimplement individualized plans of care for the patientIdentify qualifying patients and enroll in Chronic Care Management to support ER avoidanceWork closely with the triage team and APP to coordinate care for patients experiencing acute symptomsServe as a liaison between patients and the healthcare team, addressing concerns and ensuring that patient needs are met promptlyEnsure patients are educated on their healthcare needs to ensure they are part of their overall treatment plan.Maintain accurate and up- to date- patient records, documenting all interactions, care time, and outcomes in compliance with health regulationsWe Require:Graduate from an accredited School of Nursing and hold an unencumbered New York license.Nursing Experience which one has acquired clinical competence preferredExcellent assessment, communication and organizational skillsGood computer knowledgeAbility to plan, organize, develop, and interpret health data, goals, and objectives that are necessary for providing quality coordination of careBi-Lingual, English/Spanish a plusHemOnc experience preferredWhat We Offer:Salary: Starting at $32/hourBenefits: Health Insurance on day 1, Dental, Vision, Life Insurance, Short- and Long-term disability, 401k Plan, generous PTO, 8 paid holidays (2 floating), and more.Join us at NYCBS, where we are making strides in healthcare through innovative and compassionate care.Visit our website at: Nycancer.comFollow us on Facebook: NYCBS on Facebook*New York Cancer and Blood Specialists is an Equal Opportunity Employer.* #J-18808-Ljbffr Astera Cancer Care
