Requirements / Qualifications

• Certification (Copy of Valid Vocational Nurse License)
• CPR/First Aid Certification (CPR Certificate and First Aid Certificate)
• Other (Valid Driver's License and ability to provide own transportation required - please DO NOT ATTACH license copy to application)
• Proof of HS Graduation (Acceptable formats: high school diploma or equivalent; transcripts [must show name and graduation date]. If yours is a Foreign HS Diploma, please complete a Foreign HS Diploma Certification. Foreign HS Diploma Certification Form is located in the "links related to this job" to the right.)
