Responsibilities
- Administer prescribed medications and treatments in accordance with orders and facility policy
- Monitor residents' health status, vital signs, and reporting changes promptly to the care team
- Provide basic nursing care and assistance with activities of daily living to residents
- Document care provided accurately in the resident's chart or electronic health record
- Collaborate with RNs, physicians, and other staff to implement and adjust care plans
- Maintain a safe, clean, and respectful environment; adhere to infection control and safety protocols

Requirements
- Active Licensed Practical Nurse license in the applicable state
- Current CPR/BLS certification
- Prior experience in long-term care or skilled nursing settings preferred
- Strong documentation and communication skills; able to collaborate within a multidisciplinary team
- Ability to work per assigned schedule and adapt to dynamic care needs
- Ability to meet health screening and standard infection control requirements
