Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

This registered nurse position specializes in hospital case management, focusing on coordinating patient care, discharge planning, and regulatory compliance to ensure optimal patient outcomes. The role requires collaboration with multidisciplinary teams and adherence to hospital policies.

Responsibilities:

• Assess patients' needs and develop individualized care plans based on clinical and regulatory requirements.
  
• Coordinate discharge planning and facilitate necessary community or home health services.
  
• Monitor and evaluate patient progress, adjusting care plans as needed to meet recovery goals.
  
• Ensure compliance with CMS regulations, documentation standards, and hospital policies.
  
• Facilitate communication among healthcare team members, patients, and families.
  
• Complete required modules and pass all compliance exams, including EKG assessments.
  Qualifications:
  
• Valid licensure as a registered nurse with an active license in good standing.
  
• Minimum of 2 years of hospital nursing experience, including ICU, MS, PICU, SDU, PCU, IMC, or Obs.
  
• Experience with EPIC electronic medical record and case management systems.
  
• Demonstrated knowledge of CMS regulations and discharge planning processes.
  
• Current BLS certification required.
  
• Ability to pass a compliance EKG exam with up to two attempts.
