RN, Home Health Case Manager, Lombard, Downers Grove area

$38.2 - $57.3 per hour

Job Title Registered Nurse – Home Health Schedule Details Monday–Friday 8:00 AM–5:00 PM. Work Saturday and Sunday every third weekend, 1–2 on‑call days per month, and rotate for weekend on‑call. Location Lombard, Downers Grove, Westmont, Oak Brook, Willowbrook (Illinois) Compensation Pay range: $38.20 – $57.30 per hour, based on qualifications and experience. Sign‑on bonus of up to $10,000 available for new hires who are actively employed in an equivalent RN position at the time of payout. Current internal team members are not eligible. Major Responsibilities Develop and maintain a therapeutic nurse‑patient relationship throughout the healthcare continuum. Demonstrate adaptive communication styles based on individual patient needs. Provide individualized care to promote physical and psychosocial comfort. Assess patient and family readiness and involve them in care planning and implementation. Incorporate a holistic approach into the plan of care. Facilitate patients’ right to receive quality, cost‑effective care. Mobilize appropriate resources to mitigate adverse outcomes. Adapt practice to evidence‑based standards. Participate in quality and process improvement initiatives. Reduce risk and improve patient safety. Meet regulatory and mandatory unit requirements. Demonstrate commitment to patient and staff education. Assess readiness to learn and adjust educational approach accordingly. Enhance knowledge, skill, and competence of department members. Apply the nursing process as a problem‑solving model. Recognize and respond to changes in clinical situations. Implement changes to the plan of care promptly and accurately. Make sound clinical decisions for patients with complex problems. Prioritize and organize patient care, ensuring follow‑through. Initiate referrals and discharge planning with an interdisciplinary approach. Apply ethical decision making and maintain confidentiality. Delegate appropriately to unlicensed assistive personnel. Seek resources for disruptive behavior that impedes care delivery. Demonstrate respect for peers and other disciplines; collaborate as part of the care team. Participate in unit meetings and activities that improve patient care. Communicate effectively, give feedback, and resolve conflicts. Critique own practice and meet required certifications or a bachelor’s degree. Earn a minimum of 10 contact hours of continuing education per year. Licensure Registered Nurse (RN) license issued in Illinois. Valid driver’s license issued by the Division of Motor Vehicles. Education / Experience Required Graduate from an accredited nursing school. One year of medical‑surgical experience. Proficiency in RN role requirements. Earn 10 continuing education credits annually. Knowledge, Skills & Abilities Required Effective communication and interpersonal skills. Strong documentation skills. Computer proficiency. Good time‑management and organizational skills. Familiarity with patient care equipment. Current Illinois RN license. Current CPR certification. Physical Requirements And Working Conditions Ability to work weekends, holidays, and varying shifts. Ability to perform on‑call assignments. Handle multiple tasks and meet tight deadlines. Transport objects, patients, and equipment. Respond quickly to patient needs in a fast‑paced environment. Work in environments with potential exposure to hazardous conditions. Work with infectious specimens and patients. Respond promptly and access all unit areas. Lift up to 100 pounds occasionally or 50 pounds frequently. Travel throughout the Advocate service area in all weather conditions. Assist with ambulation, turning, and transferring patients up to 200 pounds. Willingness to work occasional overtime. Must be able to visit private residences that may not be accessible to individuals with disabilities. Additional Employee Information Work hours approximately 40 hours per week. Full‑time status.
