Patient Care Manager (Home Health) Location: St. Paul, MN Job Type: Full-Time Salary: $90,000-$110,000 annually (based on experience) Schedule: Monday-Friday | 8:00 AM-5:00 PM | In Office Are you a Registered Nurse with home health leadership experience? We are seeking an experienced Patient Care Manager to oversee clinical operations, support field staff, and help ensure exceptional patient care in a home health setting.
