Patient Care Specialist

Support patients with high-risk health conditions in navigating the healthcare system Collaborate with multidisciplinary care teams, including physicians, nurses, pharmacists, laboratory technologists, social workers, and educators Support chronic care management and transition-of-care processes Act as a resource for clinic staff Perform nursing duties within the scope of an RN/LVN/LPN license Work in a less structured, self-directed environment Conduct patient education, motivational interviewing, and self-management goal setting Make outbound patient telecommunications Travel throughout the assigned region and conduct home visits as needed Requirements Licensed Practical/Vocational Nurse or other credentialed health professional as approved Active and unrestricted NM or Compact State License Current BLS certification 2+ years of experience in a physician's office, clinical or hospital setting Experience related to patient education and/or motivational interviewing skills and self management goal setting Experience with outbound patient telecommunications Excellent verbal and written skills Solid interpersonal skills Ability to interact productively with individuals and multidisciplinary teams Excellent organizational and prioritization skills Ability to travel throughout the assigned region and conduct home visits as needed Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, HTN, and CRF Proficient computer skills for electronic medical records Bilingual English and Spanish preferred Core Competencies Demonstrates expertise in patient education, motivational interviewing, and chronic disease management while effectively collaborating with multidisciplinary teams. Holds a valid nursing license and BLS certification, with strong organizational and communication skills to support high-risk patients.
