Physical Therapist needed for home health setting in Barre, VT!Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!Primary Job Responsibilities: Assess a persons condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a persons needs are met.
