POSITION SUMMARY

The Senior Counselor provides comprehensive, short-term, home-based services to families involved with the Department of Children and Families (DCF). The role supports families in strengthening safety, stability, and functioning with the goal of preventing out-of-home placement, and includes training and supervision for advanced engagement, assessment, and intervention implementation. $4,000 Sign-on Bonus! Half paid upon hire and half paid at one year of service! Bilingual required in Spanish and English

EDUCATION AND EXPERIENCE / QUALIFICATIONS

BA/BS degree in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required. Previous experience working with client populations with mental health and/or co-occurring substance use disorders. Effective time management, strong verbal and written communication skills, and a valid driver's license are required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Develop case management plans or plans of care and provide direct case management services and coordination of care to assigned clients and families. Provide front‑line crisis support via phone, in‑home, or community as needed, problem‑solving and consulting with supervisors for complex cases. Interpret and adapt program guidelines and procedures to meet specific client/family needs. Make decisions based on data presented and refine methods and techniques accordingly. Determine processes and methods based on assessment of client/family presentation and strengths. Provide group, individual, and/or family counseling with appropriate supervision and coordinate treatment phases with internal and/or external service providers. Maintain records of counseling, coordination of care, and case management activities for reporting and tracking, completing all documentation in compliance with Medicaid, licensing, and regulatory requirements. Demonstrate knowledge of basic psychopathology, crisis theory, and behavioral health concerns, including relevant intervention modalities such as safety planning, parenting skills, and wraparound care. Support daily program operational functions and transport clients to appointments or linkages as appropriate. Provide services (evaluation, treatment, wraparound team facilitation, follow‑up, case management) in office and community settings with a flexible schedule to deliver services as defined by the program. Support safety planning and crisis intervention, including rotating on‑call coverage for 24‑hour availability. Participate in developing initial treatment plan, transition plan, and timeline for strategies and next steps. Show knowledge of internal and external levels of care, referral processes, and resources; make appropriate referrals in a timely manner. Meet direct service productivity goals, understand program guides and target outcomes, and adhere to practice standards and data collection protocols. Identify potential risk factors and reduce risks in the home through supervision, reinforcement of positive behaviors, trauma‑informed and relationally‑based therapeutic interventions. Attend and prepare for scheduled supervision, team meetings, staff meetings, or rounds to coordinate case management and review of treatment plans, seeking additional supervision or consultation as needed. Communicate positively and effectively with clients, families, and internal and external providers. Continue developing knowledge of the history, traditions, values, family systems, and artistic expression of served groups, using culturally appropriate methods and techniques.
