Description:
We are seeking a highly qualified and compassionate Director of Social Services to join our leadership team.
This pivotal role acts as a strategic partner, ensuring our social services programs reflect the highest standards of quality, policy, and comprehensive care for residents.

This position involves administrative responsibility for the delivery of services through the Social Service Office.
You will provide social work supervision to staff social workers and offer necessary backup when these social workers are unavailable.
Participation in the development of programs, policy, and priority setting regarding social services and resident care is essential.
You will provide direct and indirect service, addressing the social and emotional needs of residents and families at the time of admission, during treatment, and at discharge.


Key responsibilities include organizing social service programs and monitoring delivery, quality, and outcomes of services.
You will be responsible for developing departmental resident care policies and acting as the Discharge Planning Coordinator.
Remaining current with state and federal long-term care regulations is required.
You will attend scheduled meetings with administration and participate in weekly department head meetings.
Assuming an administrative role in special projects as requested is also key to the role.


Compliance with health information privacy related laws and regulations is mandatory.
You will assist residents and families with the admissions process, including obtaining signatures.
Notifying families about payment status changes and sending certified letters for non-coverage will be part of your duties.
Assisting families in understanding Advanced Directives and conducting tours of the facility to prospective clients will be expected.
Collaboration with nursing staff to determine resident/family permission for bed transfers is necessary.
You will assist families in navigating payment sources and understanding items on the admission checklist with the care coordinator.
Providing supervision to staff social workers is also part of your essential duties.
In collaboration with administration and other disciplines, participation in the development of social service programs and policies will be required.
