Why Join Our Team?Competitive Pay & Benefits Competitive Compensation DailyPay - Access Your Earned Wages Before Payday Medical, Dental & Vision Insurance 401(k) Retirement Plan Paid Time Off Employee Assistance Program Wellness Programs Employee Referral Bonuses Company-Paid Training Career Growth & Development Professional Development Opportunities Ongoing Training and Education Leadership Development Programs Career Advancement Opportunities Experience Working Across Multiple Service Systems Opportunities Across Multiple Service Lines Meaningful Work That Matters Support Individuals in Achieving Recovery Goals Connect People to Critical Community Resources Advocate for Participant Needs Strengthen Community Inclusion and Independence Partner with Families and Service Providers Join a Mission-Driven Organization What You’ll Do As a Case Manager , you will: Collaborate with multidisciplinary teams to provide holistic, person-centered services Assist in the development, implementation, and review of recovery and treatment plans Connect participants with community resources, benefits, and support services Build and maintain relationships with community agencies and service providers Deliver community-based services focused on recovery, education, and community integration Transport participants to appointments and services as needed Advocate for participants to access necessary medical, behavioral health, housing, and social supports Complete assessments, service documentation, and reports in compliance with regulatory requirements Monitor progress toward goals and support plan implementation Maintain accurate records in accordance with organizational, funder, and regulatory standards Identify service gaps and recommend solutions to improve participant outcomes Perform additional duties as assigned Who We're Looking For You may be a great fit if you are: Compassionate Organized Professional Team-Oriented Service-Focused Adaptable Strong Communicator Committed to Helping Others Qualifications Required High School Diploma or equivalent Minimum two (2) years of experience supporting individuals in behavioral health, addiction recovery, or IDD services Valid driver's license and reliable vehicle with required insurance Intermediate computer proficiency, including Microsoft Word, Excel, and data entry systems Ability to pass required background screenings, including FBI clearance if applicable Strong communication, organizational, and problem-solving skills Ability to collaborate effectively with participants, families, and community partners Ability to multitask and remain focused in a dynamic environment Ability to work effectively within a team environment Familiarity with writing progress notes and service documentation Preferred Bachelor's Degree in Human Services or a related field Minimum one (1) year of case management experience Experience supporting individuals with intellectual and developmental disabilities (ID/DD) Familiarity with creative arts-based programming Knowledge of behavioral health, IDD services, and crisis intervention Positive Behavioral Supports (PBS) knowledge and experience Understanding of state and federal regulatory requirements Certifications CPR and First Aid Certification preferred (training provided upon hire) PAC Certification preferred (training provided upon hire) Medication Administration Certification preferred (training provided upon hire) Schedule Full-Time Combination of office-based and community-based work Travel throughout the service area required Participation in trainings, meetings, and program activities as needed Physical Requirements Ability to lift up to 50 pounds Ability to bend, stand, reach, walk, and drive as needed Ability to work in office and community-based settings Ability to use standard office equipment and computer applications Ability to perform essential job functions with or without reasonable accommodation About Our Organization Resources for Human Development (RHD) is part of the Inperium family of companies.
