Key Responsibilities of the RN - Home Health

• Observe, assess, plan, implement, and evaluate nursing support for individuals in the community or facility setting
• Collaborate with healthcare providers to plan and evaluate care programs
• Coordinate communication and actions of various disciplines related to health and safety
• Manage medication ordering, monitoring, and documentation
• Transcribe and ensure transcription of medical orders
• Prepare routine health status documentation
• Supervise activities of licensed practical/vocational nurses
• Schedule and follow up on health-related appointments
• Facilitate training for health and safety topics

Requirements of the RN - Home Health

• WV RN License
• Home Health Experience
• Proficiency with HCHB
