Position Summary

We are seeking a PRN Physical Therapist (PT) to join our dedicated in-house therapy team. The Physical Therapist will evaluate, plan, and deliver therapeutic interventions to residents with mobility limitations, functional impairments, or injuries. This role supports both short-term rehabilitation and long-term care residents.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations to assess strength, balance, mobility, and functional limitations.

  • Develop and implement individualized treatment plans based on each resident’s clinical needs and goals.

  • Provide direct therapeutic interventions including gait training, therapeutic exercise, balance activities, and functional mobility training.

  • Document all evaluations, progress notes, and discharge summaries in accordance with state, federal, and facility standards.

  • Collaborate with interdisciplinary team members—OT, SLP, nursing, and social services—to ensure coordinated and effective care.

  • Educate residents, families, and caregivers on safety, mobility strategies, and the home exercise program (HEP).

  • Participate in care plan meetings and make professional recommendations based on clinical findings.

  • Follow all regulatory, ethical, and professional guidelines for physical therapy practice.

Qualifications

  • Doctorate or Bachelor/Master’s degree in Physical Therapy from an accredited program.

  • Current Physical Therapist license in the state of practice.

  • Prior experience in a skilled nursing facility (SNF) or similar setting preferred.

  • Strong assessment, clinical reasoning, and documentation skills.

  • Ability to work independently and manage a flexible PRN schedule.
