Overview
Licensed Practical Nurse (LPN) – Long-Term Care, travel assignment in New Lisbon, Wisconsin. Start date 07/06/2026 for a 13-week contract with guaranteed 36 hours per week. Weekly pay ranges from $1,461 to $1,516. Shift: PM, 2:00 pm–10:00 pm (Full Time), with every other weekend. Setting: long-term care (LTC) environment.

Responsibilities
- Administer medications to residents following the 5 Rights and document administrations accurately.
- Monitor residents and report changes in condition to the charge nurse and physician; update family members as directed.
- Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
- Assist CNAs with call lights and other care tasks to support timely resident needs.
- Maintain clear communication with physicians and families about changes in condition; participate in care planning as directed.
- Perform other duties as assigned by the supervising nurse to support resident safety and quality of care.

Requirements
- Current Wisconsin LPN license; formal LPN education completed.
- Current CPR/BLS certification.
- 1+ years of related LTC experience preferred.
- Ability to work in a long-term care setting; strong attention to detail and accuracy.
- An equivalent combination of education, training, and experience may be considered.

Education
- LPN program completion with active Wisconsin license; ongoing professional licensure as required.
