Schedule / Job Type

Contract | Days | Monday–Friday, 8:00 AM–5:00 PM (flexible); every 3rd weekend required; minor holidays as scheduled

Key Responsibilities

• Provide physical therapy services to patients in a home-based care setting within an assigned geographical region
• Evaluate and treat patients with diagnoses including orthopedic conditions, strokes, joint disorders, and spinal injuries
• Complete and maintain accurate clinical documentation using Cerner EMR
• Complete OASIS assessments as required by government mandate
• Collaborate with interdisciplinary team members including OT, Speech, and support staff
• Adhere to professional dress standards including business attire and white lab coat

Required Qualifications

• Active Physical Therapist (PT) license
• Current BLS certification
• Minimum of one year of experience as a Physical Therapist
• Demonstrated experience with OASIS documentation
• Ability to work every 3rd weekend and minor holidays as needed

Preferred Experience / Skills

• Prior experience in a home health or home-based care setting
• Familiarity with Cerner EMR platform
