Field Registered Nurse — Community LTSS Assessments
Maximus
Maximus is seeking a Field Registered Nurse to perform in-person assessments in members' homes, hospitals, and community settings to evaluate medical, functional, behavioral health, and social needs within the MassHealth LTSS program.
You will develop person-centered service plans, educate members and caregivers on available resources, advocate for vulnerable populations, and collaborate with healthcare providers and community partners to ensure high-quality, compliant care, while enjoying a
