Start Date: 07-27-2026
End Date: 10-26-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy evaluations and treatments to home health patients
- Develop and implement personalized care plans for recovery and independence
- Perform start of care, recertifications, evaluations, and daily visits
- Collaborate with clinical managers and educators for guidance and support
- Document patient progress using Homecare Homebase (HCHB) charting system
- Educate patients and families on exercises and home safety
Qualifications
- Active Physical Therapist license in Kentucky (or compact eligible)
- Minimum 1 year of experience in home health setting
- BLS certification from the American Heart Association required
- Must have experience with Home Care Home Base (HCHB) and home health
- Ability to work independently and manage a caseload of 6.5 points per day
Knowledge & Skills
- Expertise in home health therapy including SOC, ROC, recerts, and evaluations
- Proficiency with Homecare Homebase (HCHB) electronic medical records
- Strong clinical reasoning and patient assessment skills
- Excellent communication and patient education abilities
- Knowledge of common diagnoses: CHF, Orthopedic conditions, Pneumonia
