Overview Social Worker | JGS – Leavitt Family Jewish Home — $30.00 - $34.00/hour , based on experience • $3,000 Sign-On Bonus • Full-Time At JGS – Leavitt Family Jewish Home, we provide compassionate and comprehensive care and value resident care and employee appreciation. Responsibilities Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs. Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life. Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care. Connect residents and families with community resources and post-discharge support services to promote successful care transitions.
