This role involves providing essential support to individual clients as part of personalized care teams with independent assignments, including regular travel to client homes and appointments within the local area. Responsibilities

• Assist with personal care and daily routines
• Support client mobility and transfers
• Provide companionship and emotional support
• Help with meals, light housekeeping, and medication reminders
• Transport clients to appointments and errands as needed
• Document care and communicate with care teams using electronic health records and mobile apps
• Maintain a safe and supportive environment

Preferred Qualifications

• 1+ years of caregiving and CNA experience
• High school diploma or equivalent
• AZ Fingerprint Clearance Card
• CPR/First Aid Certification
• Skills in patient care, mobility assistance, medication reminders, and companionship
• Proficient in documentation, reporting, communication, and time management
• Reliable transportation and strong sense of compassion
