Home Health Aide Full-Time Willmar, MN, US 5 days ago Requisition ID: 10686 Salary Range: $16.07 To $18.10 Hourly BETHESDA AT HOME HOME HEALTH AIDE/HOMEMAKER JOB DESCRIPTION Job Summary: The Home Health Aide/Homemaker is responsible for providing personal care and designated health related services designed to maintain the client’s physical and emotional well-being. In addition to aiding with light housekeeping duties to assigned clients. Job Relationships: Reports to: Home Health Director Secondary Reports to: RN Case Managers Internal Relationships: Home Health Aides, and other Homemakers External Contacts: Clients and Family Members Hours of Work: As assigned Monday – Friday, 8:00 AM – 4:30 PM. Other hours as directed and able. Must be flexible. Dress Code: Besthda branded scrubs and name badge. Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.) Education: High school graduate or equivalent. Home health aide/homemaking-Successful completion of an approved competency evaluation. Hospice aid: Approved Certified Nursing Assistant program with competency evaluation or equivalent. Personal: Ability to foster independence, self-respect and self-worth in each client by consistent understanding and kindness. Proficient in basic communication skills. Must be able to read and understand instructions. Must write legibly. Must follow oral and written directions consistently. Valid driver’s license, vehicle, and proof of insurance. Job Knowledge: Must have knowledge of personal cares. Must have knowledge of housekeeping, laundry, meal preparation, and shopping Physical Demands of the Job: Frequent walking, standing, twisting, bending, stooping, pushing, and pulling. Occasional squatting, kneeling, sitting and reaching above shoulder level. Lift weights 20 to 40 pounds frequently. Lift weights to 50 pounds occasionally. Transfers clients with weight bearing abilities weighing up to 250 pounds Principle Job Responsibilities: Responsibility: Assist and perform tasks to provide personal care as assigned by RN Case Manager and/or through client care plan. Perform light housekeeping tasks to maintain clean and safe home environment Tasks: Follows all policies and procedures including infection control. Assists with baths/showers. Assists with transfers and ambulation. Assists with toileting. Assists with general personal cares: foot and nail care, shaving, oral care, hair care. Assists with or provides ROM/PROM and other rehabilitative exercise as instructed. Appropriately responds to emergency situations and changes in client condition. Reminds client of medication if directed on care plan. Does not administer medication unless trained and directed by RN Case Manager and the care plan. Other duties as assigned in care plan Responsibility: Performs assigned light housekeeping tasks to maintain clean and safe environment. Tasks: Dusting Sweeping and Mopping General cleaning of kitchen and bathroom Washing and drying dishes Cleaning refrigerator and oven (not heavily soiled) with soap and water only Responsibility: Prepare meals in accordance with supervisor’s direction and client’s care plan. Tasks: Prepare food Properly store food Clean-up Responsibility: Perform laundry tasks to maintain a clean and healthy home environment. Tasks: Collect and sort laundry Wash and dry clothing and linens Iron, fold and put clothing away Responsibility: Assist with or complete shopping and other errands which may include taking client to appointments. Tasks: Assist to write down shopping list Assist or do shopping and other errands Take client to appointments as directed Handle and document use of client’s money Responsibility: Complete necessary documentation to high level of accuracy. Tasks: Maintain accurate and concise notes of services and observations Record time spent with client Record mileage Submit documentation on a weekly basis by Friday afternoon or 8:00 AM Monday. Complete EVV Documentation Professional and Personal Growth: Report for and leave the work area on a timely basis. Participate in annual performance evaluation process. Write yearly personal goals. Participate in all required in-service programs at Bethesda including infection control. Attend at least 12 hours of continuing education each calendar year. Maintain professional conduct and a pleasant, friendly attitude towards staff, clients, and family members. Be willing to learn and change to meet needs that arise.
