St. Clair County Community Mental Health is seeking a full-time Case Manager in Port Huron, MI to provide direct services to individuals served, assess needs, and guide access to Medicaid, Medicare, Food Benefits, and Housing Benefits. You will monitor plans, document progress, and coordinate with schools, physicians, and community agencies. Ideal candidates have a bachelor’s degree in a related field, a valid driver’s license, and QMHP/QIDP/CMHP eligibility with experience serving ID/DD #J-18808-Ljbffr St. Clair County Community Mental Health
