Full-Time | Evening Shift | One‑on‑One Care

???? Schedule: Monday-Friday | 3:00 PM - 11:00 PM

Community Care, Inc. is looking for a compassionate and dedicated LPN to provide 1:1 private duty nursing care for a wonderful 32‑year‑old female adult . This is a great opportunity to build meaningful connections, deliver personalized care, and see the impact of your work every single day-all in a supportive home setting.

???? What You'll Do

Assist with Activities of Daily Living (ADLs)
Perform trach/vent care/monitoring
Administer medications safely and accurately
Communicate clearly and compassionately with the care team and family
Deliver exceptional, family‑centered client care

✅ What You'll Need

Current LPN (or RN) license in the state of Pennsylvania
Graduate of an accredited nursing program
At least 6 months of nursing experience (home health preferred, but not required)
CPR certification
Reliable transportation and a valid driver's license
A caring heart, strong independence, and a passion for client care
