Care Coordinator (EN)

About Us Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support --- partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential. The Role We’re looking for a Care Coordinator to be the connective thread between patients, families, clinicians, and physicians. This is a hybrid role --- most coordination and documentation work is done remotely, with occasional in-person visits or meetings as needed. What You’ll Do Coordinate scheduling of clinical and support services across the care team. Serve as the primary point of contact for patients and families regarding services. Track authorizations, re-certifications, and service frequency requirements. Communicate with physicians and clinicians regarding changes to the plan of care. Monitor case status and flag gaps in service or documentation. Maintain accurate records in company systems, including HHAeXchange and Salesforce. Support case conferences and interdisciplinary team communication. Participate in firm accreditation activities and surveys. What You Bring Experience in home care, healthcare coordination, or case management preferred. Strong organizational skills and comfort managing multiple cases at once. Clear, professional communication with patients, families, and clinicians. Proficiency with Google Workspace and care management systems.
