JOB TITLE: Family Medicine Nurse Practitioner
POSITION SUMMARY: The Family Medicine Nurse Practitioner (NP) provides comprehensive, patient-centered primary care to patients across the lifespan. The NP is responsible for assessing, diagnosing, treating, and managing acute and chronic health conditions while emphasizing preventive care, health education, and wellness. This position partners with patients, families, and the interdisciplinary care team to develop individualized care plans, promote healthy outcomes, and ensure continuity of care. ESSENTIAL FUNCTIONS:

- Conduct comprehensive health histories, physical examinations, and assessments while maintaining patient dignity and privacy.
- Clearly communicate diagnoses, treatment options, medications, health risks, and recommendations in a manner patients can understand.

- Maintain accurate, timely, and confidential medical records in accordance with organizational and regulatory requirements.
- Provide culturally responsive, equitable, and patient-centered care.
- Recognize and appropriately respond to changes in a patient's condition, including urgent and emergent medical needs.
- Provide evidence-based psychiatric evaluation, diagnosis, treatment, and medication management within the scope of practice.
- Develop, implement, and regularly evaluate individualized treatment plans based on patient needs and clinical outcomes.
- Monitor medication effectiveness, adherence, side effects, potential interactions, and overall treatment response.
- Maintain current knowledge of behavioral health best practices, psychiatric medications, clinical guidelines, and regulatory requirements.
- Participate in quality improvement initiatives, clinical case reviews, and interdisciplinary team meetings.
- Maintain complete and timely documentation in the electronic health record in accordance with organizational and regulatory standards.
- Identify opportunities to improve patient outcomes, access to care, clinical workflows, and the overall patient experience.
- Identify opportunities to improve access, efficiency, quality, and continuity of primary care services.
- Utilize technology, telehealth, electronic health records, and other appropriate tools to enhance patient care and engagement.
- Support innovative approaches to preventive care, chronic disease management, and population health.
- Adapt care strategies to meet the changing health needs of patients and the communities served.
- Collaborate with the care team to develop creative solutions to barriers that impact access to healthcare and health outcomes.
- Participate in the development and implementation of new clinical programs, workflows, and initiatives as appropriate.
- Embrace change and demonstrate flexibility in responding to evolving patient, community, and organizational needs.
- Develop long-term, therapeutic relationships with patients and families across the lifespan.
- Engage patients in shared decision-making and encourage active participation in their healthcare.
- Provide education regarding diagnoses, medications, preventive health, nutrition, lifestyle, and self-management strategies.
- Partner with patients and families to establish realistic and meaningful health goals.
- Collaborate with physicians, nurses, behavioral health professionals, pharmacists, specialists, care coordinators, and other members of the interdisciplinary team.
- Coordinate transitions of care, referrals, follow-up services, and community resources to promote continuity of care.
- Build relationships with community partners and organizations that support the health and well-being of patients.
- Advocate for patients and help identify and address social and other barriers that may affect access to healthcare.
- Promote a welcoming, inclusive, and supportive care environment for all patients.
- Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
- Doctor of Nursing Practice (DNP) or other doctoral-level education.
- Additional education or training in family medicine, primary care, population health, or a related specialty.
- Current, unrestricted Nurse Practitioner license in Washington.
- Current prescriptive authority in accordance with state and federal requirements.
- Current DEA registration.
- Current BLS certification.
- Current national board certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
- Experience providing primary care services in an outpatient, family medicine, community health, or similar clinical setting.
- Experience conducting comprehensive patient assessments, diagnosing common medical conditions, and developing treatment plans.
- Experience managing acute and chronic health conditions across diverse patient populations.
- Experience collaborating within an interdisciplinary healthcare team.
- Strong clinical assessment, diagnostic reasoning, and decision-making skills.
- Knowledge of evidence-based primary care, preventive medicine, and chronic disease management.
- Strong medication management and patient education skills.
- Excellent communication and interpersonal skills.
- Ability to establish trusting relationships with patients and families.
- Strong organizational, time-management, and documentation skills.
- Ability to prioritize multiple clinical needs in a fast-paced environment.
- Ability to work effectively with patients from diverse backgrounds and demonstrate cultural humility.
- Proficiency with electronic health records and healthcare technology.
- Commitment to quality, patient safety, continuous improvement, and collaborative care.
