Hospice Admissions RN: Patient-Centered Care Coordinator

VIA Health Partners is a community-based hospice and palliative care provider in North Carolina seeking an experienced RN for admission coordination. You will lead the initial nursing assessment, develop a plan of care with the interdisciplinary group, and educate patients and families at admission.
