Summary Responsible for presenting hospice services at the point of admission (home or community) for individuals and their families. Initiates individualized plan of care by working with patient, family, physician, and hospice interdisciplinary group to anticipate patient needs. Serves as the initial RN member of the interdisciplinary group to develop specific plan of care goals. Essential Functions Effective Coordination of Patient/Caregiver Care Conducts admission nursing assessment to identify patient/caregiver needs.
Develops plan of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions, and specifically addresses patient/caregiver education.
Determines individualized goals and interventions on the plan of care to meet physical, emotional, and spiritual needs.
Initiates and maintains a liaison relationship with healthcare providers to coordinate multidisciplinary care.
Consults with patient, caregivers, physicians, facility staff, and interdisciplinary team when initiating care.
Communicates closely with staff nurse assigned as case manager to ensure clarity of care.
Maintains productive and professional relations with external agencies and healthcare providers.
Delivery of Health Services Confirms scheduled visits in advance based on prioritized judgment of case load needs.
Prepares for visits, anticipates care needs, reviews referral information, and obtains necessary materials.
Makes visits to evaluate appropriateness for hospice care, following established admission procedure including consents, medication evaluation, symptom management, teaching, and documentation.
Provides interventions under direction of a physician and identifies new problems or needs at the admission visit.
Applies principles of infection control and ensures universal precautions.
Makes cost‑effective use of resources to ensure delivery of healthcare.
Plans workday to respond to priorities and minimize travel time.
Uses VIA Health Partners patient‑related protocols, standing orders, and guidelines including Hospice Pharmacia MUGs, criteria for oxygen use, and specialized treatments as appropriate.
Coordinates with Physician of the Day (POD) to ensure eligibility and provide assessment findings for Initial Narrative Certification of Terminal Illness.
Demonstrates nursing care procedures and utilizes teaching materials, tools, and written information appropriately.
Provides education to facility staff if admitting a patient in a HART facility.
Collaborates with VIA Health Partners Medical Social Worker to address psychosocial needs at initial interdisciplinary care plan development.
Clinical Documentation Maintains current clinical records by synchronizing laptop information multiple times a day.
Completes and submits all clinical documentation per VIA Health Partners guidelines.
Initiates and maintains current medication profile as directed by physician, documenting medication teaching details.
Communicates and coordinates with Hospice Pharmacia regarding appropriate medications.
Initiation of Nursing Assistant Care Plan Develops Nursing Assistant care plans in accordance with patient needs and scope of practice.
Submits Nursing Assistant care plans to administrative assistant for team communication the next business day after admission.
Participates actively in process improvement.
Ensures VIA Health Partners professional reputation is maintained and projected.
Continues professional development and stays abreast of current healthcare trends.
Strives for positive relationships with physicians’ offices, hospitals, assisted living communities, and nursing homes.
Qualifications Minimum Qualifications Diploma or Associate’s Degree in nursing (required). Bachelor’s Degree preferred.
Current, active, and unrestricted NC/SC license as Registered Nurse (RN).
Oncology nurse certification if required for chemotherapy administration.
Minimum 2 years related experience, including direct care of patients with life limiting illnesses (hospice, palliative, family medicine, internal medicine, geriatrics, oncology, home health care, medical‑surgical).
Experience in environments focusing on pediatric population is required for Kids Path Nurse with BCLS (CPR) training for both adult and child/infant.
Computer proficiency required.
