Behavioral Health Social Worker / Therapist – Program & Support Planning

Avento Health is recruiting a Behavioral Health Social Worker / Therapist for a community-based developmental disability services organization in Springfield, Illinois. This position supports adults with intellectual and developmental disabilities through individualized program planning, clinical documentation, behavioral-health support, therapeutic services, and interdisciplinary collaboration.

Compensation

$71,400–$86,100 annually.

Position Overview

This role has a strong program-writing and support-planning focus. The clinician will develop, review, and revise individualized program and support plans; translate identified needs and goals into clear interventions and support strategies; monitor progress; complete required clinical documentation; and collaborate closely with direct support staff, QIDPs, leadership, guardians, healthcare professionals, and other members of the interdisciplinary team.

The clinician will also provide therapeutic and behavioral-health support and help ensure that services are person-centered, clinically appropriate, accurately documented, and aligned with applicable organizational and regulatory requirements.

Key Responsibilities

• Write, review, evaluate, and update individualized program and support plans based on individual needs, goals, previous performance, outcomes, and interdisciplinary recommendations.
• Develop clear steps, procedures, interventions, objectives, and support strategies designed to help individuals achieve identified goals.
• Modify plans as individual needs, behaviors, circumstances, or outcomes change.
• Develop measurable outcomes and monitor progress using tracking data, progress notes, rating scales, observations, and other available information.
• Complete IPP/ISP documentation, progress notes, reports, assessments, service-planning documentation, and other required clinical records.
• Write and update behavioral support plans when clinically appropriate.
• Participate in intake activities and develop initial behavioral or support plans for newly admitted individuals.
• Complete mental-health and behavioral assessments as appropriate to the clinician’s scope and credentials.
• Provide individual therapeutic services based on identified needs and the individual’s treatment or service plan.
• Complete required therapy, QMHP, and other applicable clinical documentation.
• Review individual records, tracking sheets, goals, progress notes, and supporting documentation for completeness, accuracy, quality, and compliance.
• Conduct clinical or program audits and identify areas requiring correction or improvement.
• Monitor incident reports and contribute to plans addressing resolution, risk reduction, and ongoing service needs.
• Participate in IPP/ISP meetings, interdisciplinary team meetings, guardian or family meetings, intake and discharge planning, and other case conferences.
• Collaborate with QIDPs, program leadership, direct support professionals, behavioral-health professionals, medical providers, and other members of the support team.
• Train and coach direct-support and program staff on individualized goals, behavioral interventions, support strategies, documentation requirements, and implementation of program plans.
• Evaluate whether interventions and supports are being implemented as designed and provide retraining or corrective guidance when necessary.
• Support individuals in developing independence, functional skills, appropriate social interactions, community participation, and self-reliance.
• Participate in behavior-management and human-rights processes when appropriate.
• Serve as a clinical liaison with hospitals and outside providers when individuals require psychiatric or behavioral-health services.
• Participate in psychiatric hospitalization, discharge, transition, and placement planning as needed.
• Maintain required documentation in accordance with applicable organizational, regulatory, licensing, and funding requirements.

Required Qualifications

• Master’s degree in Social Work or a closely related behavioral health, psychology, counseling, or human-services field.
• At least 1 year of directly related professional experience in behavioral health, developmental disabilities, social services, or a similar human-services setting.
• Experience developing or contributing to individualized service plans, treatment plans, behavioral plans, support plans, or comparable clinical or program documentation.
• Strong clinical writing, documentation, assessment, and case-management skills.
• Ability to translate assessment findings and individual needs into practical, measurable goals and support strategies.
• Strong interpersonal and interdisciplinary collaboration skills.
• Ability to maintain accurate records and meet applicable documentation and regulatory requirements.
• Valid driver’s license and current automobile insurance when travel between service locations is required.

Preferred Qualifications

• MSW.
• Illinois clinical licensure or another relevant professional credential appropriate to the individual’s discipline and responsibilities.
• LCSW, LCPC, BCBA, PsyD, or comparable behavioral-health credential.
• 5+ years of directly related experience.
• Experience supporting adults with intellectual or developmental disabilities.
• Experience writing IPP, ISP, behavioral support, treatment, or individualized program plans.
• Experience conducting mental-health or behavioral assessments.
• Experience training DSPs or other direct-care staff.
• Experience with community-based residential services.
• Familiarity with behavioral-health, human-rights, incident-management, and regulatory processes.

Work Environment

This position supports individuals receiving community-based developmental disability services and may involve work in residential homes, administrative offices, community settings, and other service locations. The clinician should be comfortable interacting directly with individuals, staff, guardians, healthcare professionals, and interdisciplinary team members and responding to changing behavioral, clinical, or programmatic needs.

Scheduled hours may vary based on program needs, meetings, individual needs, and urgent situations.
