Now Hiring: MSW Social Worker - Assertive Community Treatment (ACT)

Location: Manhattan, NY
Schedule: Monday-Friday | 9:00 AM - 5:00 PM
Work Environment: 75% Field-Based | 25% Office-Based
Employment Type: Full-Time

We are seeking a compassionate, community-focused MSW Social Worker to join our multidisciplinary Assertive Community Treatment (ACT) team serving clients throughout Manhattan.

Responsibilities

As an MSW Social Worker, you will:

• Coordinate, lead, and document activities related to Co-Occurring Disorders (COD)
• Manage an assigned caseload of ACT participants
• Complete comprehensive psychosocial and substance use assessments
• Develop individualized treatment plans and measurable recovery goals
• Provide clinical expertise on substance use disorders and harm reduction strategies to the ACT team
• Connect clients with psychiatric care, medication management, rehabilitation services, benefits, housing resources, and community supports
• Research community substance use treatment resources and build strong partnerships with provider organizations
• Accompany clients to medical, behavioral health, and community appointments as needed
• Conduct community-based visits in homes, hospitals, shelters, residential facilities, and other community settings throughout Manhattan
• Maintain communication with family members and support systems regarding treatment progress
• Collaborate with healthcare providers and community agencies to ensure coordinated, comprehensive care
• Maintain timely, accurate, and compliant documentation in accordance with program standards
• Participate in a rotating after-hours on-call schedule
• Attend team meetings, case conferences, supervision sessions, and professional development trainings
• Work collaboratively with an interdisciplinary ACT team to provide comprehensive, recovery-oriented care
• Perform additional responsibilities and special projects as assigned

Qualifications

Required

• Master's Degree in Social Work (MSW)
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred or eligible for licensure (adjust based on your requirements)
• Experience working with individuals living with Serious Mental Illness (SMI) and co-occurring substance use disorders
• Strong assessment, case management, and care coordination skills
• Proficiency in Microsoft Office, including Word and Outlook
• Ability to learn and utilize the AWARDS Electronic Health Record (EHR) system and other electronic databases

Preferred

• CASAC Certification
• Experience conducting substance use assessments, including tobacco, alcohol, and opioid use disorders
• Knowledge of harm reduction principles and best practices
• Experience providing integrated mental health and substance use treatment
• Familiarity with evidence-based practices including:
  • Trauma-Informed Care
  • Motivational Interviewing
  • Stage-Wise Treatment
• Previous ACT, community mental health, or intensive case management experience

Work Environment

• Monday-Friday | 9:00 AM - 5:00 PM
• Approximately 75% field-based and 25% office-based
• Travel throughout Manhattan to client homes, hospitals, shelters, residential programs, and community agencies
• Participation in an after-hours rotating on-call schedule
