RN - Clinical Coordinator

MDS Coordinator - Skilled Nursing Facility Assignment Duration This is a 13-week contract position. Schedule Day shift, 5x8 hours, from 08:30 to 17:00. Job Description The MDS Coordinator is responsible for completing assessments and managing care plans in a Skilled Nursing Facility. This role requires extensive knowledge of the MDS process and ongoing interaction with residents in need of short-term rehabilitation. The ideal candidate will work in a team-oriented environment, performing comprehensive evaluations and ensuring adherence to regulatory standards. Responsibilities Complete MDS assessments and develop baseline care plans. Conduct care plan meetings and facilitate care plan development. Manage the submission and validation of assessments. Oversee certification and recertification processes. Educate team members on MDS changes and conduct utilization reviews. Review admission referrals and coordinate preauthorization for insurance. Required Experience / Certifications / Licensure Bachelor of Science in Nursing (BSN) required. Minimum of 3 years of current MDS experience is required. BLS certification required.
