Case ManagerWe are seeking a dedicated Case Manager to join our team and provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program. This role serves as a liaison among patients, families, nursing staff, physicians, multidisciplinary teams, and healthcare benefactors to optimize outcomes. You will coordinate a continuum of care for a defined patient population from pre-admission through post-discharge, ensuring a smooth hospitalization process.Job SummaryThe Case Manager will be responsible for monitoring the appropriateness and timeliness of patient care, ensuring the interdisciplinary care plan aligns with each patient's clinical course, continuing care needs, and covered services. The role focuses particularly on complex patients, frequent emergency department utilizers, chronic pain patients, substance abuse patients, and individuals experiencing homelessness.Essential FunctionsCoordinate patient care with other disciplines within the care team.Monitor diagnostic testing, treatments, procedures, and other aspects of patient care in acute care settings.Engage with physicians regarding resource utilization, treatment plans, estimated length of stay, and discharge planning.Collaborate with acute care case managers to ensure appropriate ongoing care for patients.Manage patient transfers to tertiary centers, including the needed transfer of patient information for continuity of care.Collect and oversee data for regulatory and accreditation compliance.Conduct focus study reviews of frequent emergency department visitors based on previous and current admissions.Perform additional job-related duties as assigned.Job RequirementsEducation and Work ExperienceBachelor's Degree in Nursing (BSN): PreferredExperience in a care management role: PreferredLicenses/CertificationsRegistered Nurse (RN) licensure in the state of practice: RequiredCase management certification: PreferredBasic Life Support (BLS) certification from an approved vendor: RequiredThe organization is committed to the safety and well-being of associates and patients.
