Work Environment

This is a primarily onsite role focused on home care, where you'll have the opportunity to meet patients in their own environments. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing detailed assessments, and documenting diagnostic results.

• Clearly explain evaluation findings and collaboratively establish treatment plans with patients and their families, integrating their goals and preferences into the rehabilitation process.

• Implement and monitor treatment protocols tailored to the individual patient's needs, continually adapting approaches based on their progress and changing health conditions.

• Consult with other healthcare professionals when necessary, issuing referrals or modifications to treatment plans in response to observed patient changes.

• Engage in comprehensive discharge planning, ensuring patients have clear follow-up care instructions, while maintaining accurate documentation and billing for all services rendered.

• Demonstrate effective time management to meet daily productivity expectations, including maintaining timely schedules and entering data accurately into our health records system.

• Contribute to departmental goals, ensuring compliance with local, state, and federal regulations while participating in practice improvement initiatives.

• Actively pursue continuing education opportunities to enhance your professional development and keep abreast of the latest therapeutic techniques and best practices in physical therapy.

• Support initiatives that enhance patient access to care, working collaboratively with team members to eliminate barriers and improve patient experiences.

Patient Population / Client Focus

You will work primarily with orthopedic patients in an outpatient setting, focusing on recovery from surgeries, injuries, and chronic conditions. Tailoring treatment to address both physical ailments and the emotional impacts of rehabilitation is key, as you'll build relationships with your patients and their families throughout their recovery journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.

• A valid physical therapy license to practice in the state of Florida is required.

• Current certification in Basic Life Support (BLS) from the American Heart Association is mandatory.

Preferred Experience

While prior work experience in a clinical setting is not required, it is preferred that applicants have some exposure to outpatient rehabilitation or home care environments. Enthusiasm for learning and developing practical skills to enhance patient care is vital to success in this role.
