Care Management Assistant

Summa Health - Akron Campus Summary The Care Management Assistant (CMA) provides support to members of the Care Management team. As part of the discharge planning team, the CMA participates in care coordination and transition planning efforts, facilitating the efficient movement of patients to post-acute facilities such as Skilled Nursing Facilities (SNF), Long-term Acute Care Hospitals (LTACH), Inpatient Rehab Facilities (IRF) and Intermediate Care Facilities (ICF). The role includes receiving task triggers to create referrals, transmitting them via a HIPAA‑secure platform, initiating pre-certification, and ensuring compliance with federal and state regulations. Responsibilities Assist in discharge planning and coordination with interdisciplinary teams, providers, allied health staff, patients, caregivers, and visitors. Handle task triggers to create and transmit referrals and initiate pre-certifications as directed by RN Case Manager, Utilization Management RN, or Social Worker. Serve patient rights notifications related to Discharge Planning and Utilization Management. Collaborate with external community providers, facility admissions staff, and insurance companies. Maintain compliance with federal and state regulations concerning patient rights and documentation. Minimum Qualifications Associate degree or equivalent combination of education and experience. Experience & Training Preferred Basic experience using Microsoft Office products. Knowledge of medical terminology. At least one year of experience working with an electronic health record (EHR). Previous insurance and prior authorization experience. Other Skills, Competencies And Qualifications Excellent organizational and decision‑making skills. Strong communication skills (verbal and written). Ability to prioritize and work in a fast‑paced environment with attention to detail. Capacity to complete multiple tasks simultaneously. Maintains confidentiality of all patient information. Basic computer skills and navigation of electronic platforms. Knowledge of CPT and ICD-10 codes. Interpersonal skills for effective interaction with patients, families, caregivers, peers, and supervisors. Ability to interact with patients/customers with respect and dignity. Level Of Physical Demands Sedentary work; may lift up to 10 lbs occasionally or handle negligible force frequently.
