Job Title: Travel Registered Nurse (RN) – Case Management
Location: Eureka, CA
Duration: 13 Weeks
Shift: Days (Schedule TBD)

Responsibilities

• Coordinate and manage patient care plans across the healthcare continuum
• Assess patient needs and develop individualized care strategies
• Collaborate with physicians, nurses, social workers, and other healthcare professionals
• Facilitate discharge planning and ensure appropriate follow-up care
• Monitor patient progress and resource utilization
• Document patient care activities accurately in Epic EMR
• Float between like units based on facility needs

Requirements

• Active California RN License
• Minimum 2 years of Case Management RN experience
• Acute Care experience required
• Epic charting experience preferred
• Strong communication, assessment, and care coordination skills
• First-time travelers accepted with clinical approval
