Schedule: Every Other Weekend Required

Required Experience

• Active RN license
• Minimum 2 years of Acute Care Hospital Case Management experience
• Must have performed:
  • Care Coordination
  • Discharge Planning
  • Utilization Review/Case Management
• Experience must be current within the last year
• Experience with:
  • HMOs, IPAs, and managed care plans
  • Advance Directives
  • POLST
  • Protective Services processes
• Epic charting experience required
• Travel experience preferred
