Role Description This full-time, on-site Licensed Social Worker role is based in the Cincinnati Metropolitan Area and focuses on supporting seniors who receive services through Meals on Wheels. Day-to-day responsibilities include conducting assessments, developing care plans, and coordinating services to enhance clients’ independence and quality of life. The Licensed Social Worker collaborates with interdisciplinary team members, community agencies, and families to address social, emotional, and resource-related needs. The role also involves maintaining accurate documentation, monitoring client progress, advocating for services, and responding to changes in client circumstances to ensure continuity of care. Qualifications Current licensure as a Social Worker (LSW or equivalent) in the state of Ohio; eligibility for licensure or reciprocity in Kentucky preferred. Demonstrated experience in senior services, gerontology, or community-based social work, preferably in a nonprofit or human services setting. Strong skills in client assessment, care planning, and case management to support older adults with diverse needs. Ability to build rapport with clients and families, using trauma-informed, person-centered, and culturally responsive practices. Proficiency in documentation, electronic case records, and basic computer applications (e.g., email, word processing, data entry). Effective communication skills, including clear verbal and written communication and active listening with clients, colleagues, and partners. Strong organizational and time-management abilities to manage caseloads, prioritize tasks, and meet program timelines. Commitment to ethical practice, confidentiality, and compliance with agency policies, relevant regulations, and professional standards. Experience collaborating with community resources, healthcare providers, and social service agencies to coordinate client support. Bachelor’s degree in Social Work or related field required; Master’s degree in Social Work (MSW) is an asset.
