Job Description Duties Provide skilled nursing care to patients in their homes Assess patient needs and develop individualized care plans Administer medications and treatments as prescribed by the physician Monitor patient vital signs and response to treatment Educate patients and their families on proper healthcare techniques and disease management Coordinate with other healthcare professionals to ensure comprehensive care for patients Document patient assessments, interventions, and outcomes accurately and timely Skills Strong knowledge of human physiology and anatomy Proficient in ostomy care and management Experience with wound care Ability to assess and prioritize patient needs Knowledge of OASIS standards and scoring Familiarity with creation and management of plan of care Competent in overseeing nursing care Skilled in administering injections and IV safely and accurately Understanding of safe discharge planning Proficiency in home health charting Requirements Valid Registered Nurse (RN) license in the state of California Minimum 3 years of experience as a Registered Nurse, preferably in home health or a related field Excellent communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team Strong critical thinking and problem-solving abilities Flexibility to adapt to changing patient needs and schedules Benefits Salary based on experience and qualifications Comprehensive healthcare benefits package, including medical, dental, and vision insurance Retirement savings plan with employer match Paid time off Continuing education opportunities for professional growth If you are a compassionate and dedicated Registered Nurse seeking a rewarding opportunity to provide high-quality care to patients in the comfort of their own homes, we encourage you to apply.
