Job Title: Home Health Speech-Language Pathologist (SLP)
Location: Mount Angel, OR & Surrounding Areas
Start Date: ASAP
Shift: 3x8 Hour Days (Wednesday–Friday, 8:30 AM – 5:00 PM)
Schedule Note: 4x10s or 3x12s schedules are not available
We are seeking a dedicated Home Health Speech-Language Pathologist (SLP) to provide one-on-one patient care in a home health setting. This role focuses on delivering high-quality speech therapy services, managing patient caseloads efficiently, and ensuring accurate documentation using standardized systems.
Key Responsibilities- Provide speech-language therapy services to patients in home health settings
- Manage a caseload of up to 6 patients per day (productivity standard: 30 points)
- Perform patient assessments, develop care plans, and track progress
- Complete accurate and timely documentation using required systems
- Travel to patient homes and healthcare facilities within the assigned region
- Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
- Maintain compliance with healthcare regulations and organizational standards
- Minimum 1 year of skilled home health experience
- Active SLP licensure (or pending license accepted)
- BLS (Basic Life Support) certification required
- Experience with OASIS documentation ( required )
- Valid driver’s license and reliable transportation
- COVID-19 vaccination (primary series + one booster required)
- Experience with EPIC charting system
- Strong organizational and time-management skills
- Ability to work independently in a home health environment
- Home Health setting
- Business casual attire or scrubs permitted
- No weekend or on-call requirements
- No floating between units
- Block scheduling not available
- Local candidates accepted (within 50–75 miles)
- Travel required within assigned territory (typically optimized for minimal drive time)
- Occasional travel radius may extend up to 50+ miles
- Mileage reimbursement provided at standard rate
- First-time travelers are welcome
- Candidates must be comfortable working independently and managing schedules efficiently
