Licensed Practical Nurse (LPN) - Part-Time

Location: Augusta, GA and surrounding CSRA Schedule: Part-Time | Flexible Hours | Daytime Availability Preferred

Executive Home Care of Augusta is a veteran-owned, concierge-level home care agency seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team. We help seniors across the CSRA maintain their independence and remain safely at home - and we do it through personalized, unhurried, relationship-based care, not a rotating caseload.

What You'll Do

As an LPN, you'll serve as a clinical resource for our clients, caregivers, and leadership team by:

• Conducting nursing and wellness visits
• Monitoring client health status and documenting observations
• Performing medication reconciliation and medication education
• Assisting with chronic disease education and wellness coaching
• Communicating changes in client condition to physicians and family members
• Supporting care plan development and updates
• Training and mentoring caregivers on client-specific care needs
• Assisting with hospital-to-home transitions
• Maintaining accurate clinical documentation
• Collaborating with our Care Managers to ensure exceptional client outcomes

Qualifications

• Current Georgia LPN license in good standing
• Minimum one year of nursing experience (home care, geriatrics, hospice, assisted living, rehabilitation, or skilled nursing preferred)
• Strong assessment and communication skills
• Compassionate, patient-centered approach
• Reliable transportation and valid driver's license
• Ability to pass background screening and drug screening
• Current CPR certification

• Chronic disease management
