LPN Charge Nurse

Overview The primary purpose of LPN charge Nurse is to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current Federal, State and Local standards, guidelines and regulations that govern our facility, and as may be required by the Director of Nursing or Assistant Director of Nursing to ensure that the highest degree of quality care is maintained at all times. Schedule Monday-Friday with every other weekend Administrative Functions Along with the Unit Manager, ensure written Policy and procedure is followed by all nursing personnel on the unit. Ensure nursing staff and C.N.A’s are following their respective job descriptions. Perform administrative duties such as completing the transfer, monthlies, nurse’s notes, reports, etc., as necessary. Give/receive the nursing report upon reporting in and ending shift duty hours. Co-responsible for knowledge of all Infection Control policies. Co-responsible for knowledge of all safety and fire regulations. Attend in-service programs, both in and out of the facility, to increase professional competence. Co-responsible for knowledge of all personnel policies. Perform other administrative duties delegated by 1 Wynantskill Unit Manager. Care Planning & Treatment Responsibilities Ensure MD/NP books are reviewed after visits and given to ward clerk for filing. Provide direct nursing care as necessary. Notify residents attending physician and next‑of‑kin when there is a change in the resident’s condition. Obtain sputum, urine and other lab tests as ordered and review results for abnormalities. Take and record blood pressures, etc., as necessary. Monitor seriously ill residents as necessary. Administer medications and treatments and document same as necessary. Make daily resident visits to observe and evaluate the resident’s physical and emotional status. Make rounds with a physician. Administer professional services such as, but not limited to catheterisation, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, etc. Oversee and ensure completion of weights (daily, weekly and monthly). Assist with discharge planning process. Facilitate discharge on day of discharge. Other duties as assigned. New Admissions Responsibilities Ensure that rooms are ready for new admissions. Place medication MD orders for all new admissions in the event that the Admission Nurse is not in the building. Greet newly admitted residents upon admission. Escort to room as necessary. Participate in the orientation of new residents/family members to the facility. Follow up next morning to ensure all meds delivered or not delivered are addressed with appropriate hold order.
