Come talk to us about joining the Centerstone team!**JOB DESCRIPTION:**New and innovative opportunity for a Case Manager to work collaboratively and be co-located within the local Health Department. This position is designed to increase access to behavioral health treatment and support for individuals receiving medical services through the Moore County Health Department. The Case Manager will help bridge the gap between medical and behavioral health services by identifying individuals who may benefit from additional support, facilitating connections to appropriate behavioral health resources, and promoting integrated, person-centered care.**SUMMARY OF POSITION**Jump start your career in the mental health field as a Case Manager. You’ll coordinate services and care for patients – providing coaching, strategies and coordination of health and wellness resources so they can live their lives more fully. This is a great opportunity if you’ve just graduated or if you’ve had a little experience but you’re ready for more. **ESSENTIAL DUTIES & RESPONSIBILITIES*** Help patients navigate the health care system through coaching, education and support* Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness* Refers, advocates, and links client and family for additional services as identified.* Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills* Provides supportive and educational case management services to develop daily living and coping skills.* Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.* Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.* Connect with clients face to face in the office and community, telehealth, and by phone to address needs* Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.* Attend weekly staffing and team meetings and completes assessments* Assist clients with developing natural support systems.* Completes all required agency trainings to maintain 100% compliance.* Maintains departmental productivity expectations.* Attend and participate in clinical supervision. **KNOWLEDGE, SKILLS & ABILITIES** **QUALIFICATIONS** **Education Level**Bachelor’s Degree in psychology, social work or related field required. **Years of Experience**Experience with Electronic Health Record preferred. **Certification/Licensure**Valid driver’s license in good standing required. **PHYSICAL REQUIREMENTS**Standing – 10%Sitting – 90%Squatting – OccasionalKneeling – OccasionalBending – OccasionalDriving – OccasionalLifting – Occasional **DISCLAIMER**The duties and responsibilities described in this document are not a comprehensive list and additional tasks may be assigned to the employee from time to time, or the scope of the job may change as necessitated by business demands.**Time Type:**Full time**Pay Range:**$20.05--$28.10Delivering care that changes people's lives starts with our employees.
