Registered Nurse - Homecare Coordinator

Job Description Park Nicollet is looking to hire a Homecare Coordinator to join our Homecare team! Position Summary The Homecare Coordinator is responsible for planning and coordinating the delivery of individualized care and comprehensive service to patients and families. The Homecare Coordinator supports the following major Homecare processes: admission, discharge, referral, delivery of care, complaints, staff orientation, training and development, and billing. Work Schedule M-F 8am-4:30pm, every 6 th -8 th weekend. Required Qualifications Education, Experience or Equivalent Combination: Minimum 3 years’ experience in home care Registered nurse: Associate or Diploma degree Licensure/ Registration/ Certification: Current license to practice as a professional nurse in Minnesota. Knowledge, Skills, and Abilities: Excellent written and oral communication skills and ability to speak to groups. Committed to the philosophy of customer service. Must possess knowledge of reimbursement regulations. Ability to problem solve and interface with primary customers. Ability to relate to allied health professionals and to work with and through others. Demonstrate knowledge of age-related growth and development principles necessary to provide appropriate services and assure safety of patients. Preferred Qualifications Education, Experience or Equivalent Combination: PHN preferred. Previous coordinator/supervisory experience preferred.
