Director of Nursing (DON)

Directs and coordinates the clinical operations of the home health agency under the direction of the Administrator. Holds overall responsibility for the quality, continuity, and delivery of patient care services and for compliance with all federal, state, and local regulations, including Medicare Conditions of Participation and Florida AHCA licensure requirements. Manages, staffs, trains, and develops clinical personnel; executes on the agency s quality assessment and performance improvement (QAPI) program; and drives clinical excellence and positive patient outcomes. Aids the Administrator in the overall management of the agency by performing the following duties personally or through subordinate staff.

Essential Duties and Responsibilities

• Directs and coordinates all patient care services provided by clinical staff, ensuring care is delivered in accordance with the physician s plan of care and accepted standards of practice.
• Provides guidance and oversight in the assessment, planning, implementation, and evaluation of patient care, including OASIS accuracy and timeliness.
• Reviews patient charts, employee visits, and daily activity reports to assure conformance with company standards, government regulations, and professional community health standards.
• Ensures compliance with Medicare and Medicaid Conditions of Participation, Florida AHCA (Chapter 400, Part III / Rule 59A-8) requirements, OASIS submission requirements, and accreditation standards; maintains continuous survey readiness.
• Leads the Quality Assessment and Performance Improvement (QAPI) program for the branch, monitoring clinical records, statistics, outcomes, and satisfaction data, and implementing improvement initiatives as needed.
• Oversees clinical staffing, scheduling, on-call coverage, and productivity to ensure adequate and appropriate coverage across the service area.
• Recruits, hires, manages, trains, evaluates, and develops clinical and branch staff; addresses complaints and resolves problems.
• Supports revenue-cycle and reimbursement integrity through accurate documentation, coding, and utilization oversight in coordination with the Administrator and finance.
• Coordinates with physicians, therapists, referral sources, and community partners to promote continuity of care and appropriate utilization of services.
• Assists the Administrator in the overall management of the agency and attainment of agency goals and objectives; may act as alternate Administrator in his/her absence.
• Serves as a member of the management committee and participates in special projects.

Supervisory Responsibilities

Directs clinical staff professionals and administrative support personnel and is responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the company s policies and applicable laws, including interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; and addressing complaints and resolving problems.

Education and Experience

• Bachelor s degree in Nursing (BSN) preferred; associate degree in nursing considered with equivalent experience.
• Current, active licensure as a Registered Nurse in the State of Florida is required.
• A minimum of two to four years of clinical nursing experience, including at least one year of supervisory or management experience in home health care or a related field.
• Working knowledge of Medicare/Medicaid home health regulations, OASIS, and the home health reimbursement environment strongly preferred.
