We are seeking a compassionate, resourceful, and community-focused Case Manager to join our Assertive Community Treatment (ACT) Team. This role is ideal for someone who is passionate about helping individuals overcome barriers, access resources, and build stable, meaningful lives in the community. As an ACT Team Case Manager, you won't be sitting behind a desk all day. You'll work directly with clients in their homes, shelters, community settings, and wherever support is needed. You'll serve as an advocate, coach, and connector, helping individuals navigate complex systems while empowering them to achieve their own recovery goals. Pay: $46,353.60 annually Schedule: Monday‑Friday with flexible hours Benefits: 15 Paid Holidays Annually – Including Your Birthday Robust Paid Time Off Program 401(k) Employer Match up to 5% Annual Merit Increase & Bonus Opportunities Mission‑driven work that changes lives Supportive and collaborative team culture About the ACT Team The Assertive Community Treatment (ACT) Team provides intensive, community‑based services to adults living with serious mental illness, including: Schizoaffective Disorder Major Depression with Psychotic Features Co‑occurring Substance Use Disorders Our multidisciplinary team includes psychiatrists, nurses, therapists, case managers, peer support specialists, and substance use counselors working together to provide comprehensive, person‑centered care. Key Responsibilities Assist clients in securing and maintaining stable housing. Help clients apply for and maintain benefits including SSI, SSDI, Medicaid, SNAP, and other entitlement programs. Connect clients to community resources, transportation, employment services, food assistance, and other supports. Provide life‑skills coaching related to budgeting, meal preparation, hygiene, transportation, and independent living. Advocate for clients with housing providers, community agencies, healthcare systems, and government programs. Coordinate services with therapists, nurses, psychiatrists, peer support specialists, and community partners. Support clients involved with the legal system, including probation, parole, and court programs. Conduct community‑based visits in homes, shelters, and other locations. Participate in daily interdisciplinary team meetings. Maintain accurate and timely documentation in the Electronic Medical Record (EMR). Participate in rotating weekday evening on‑call coverage. Qualifications Required Bachelor's Degree in Social Work, Psychology, Human Services, or a related field. Valid Driver's License and insurability. CPR/BLS Certification. Ability to travel throughout the community to provide services. Strong organizational, communication, and advocacy skills. Preferred Master's Degree (MSW or related field). Utah licensure or license‑eligible status (CSW‑I, LCSW, ACMHC, or LCMHC). Two or more years of experience in case management, community mental health, social services, or related human services work. Experience serving adults with serious mental illness. Experience coordinating housing, benefits, and community resources. Experience working with co‑occurring mental health and substance use disorders. Mental Health Officer designation (or ability to obtain).
