Position Overview:

Registered Nurse (RN) will be responsible for conducting comprehensive resident assessments to evaluate their health needs, monitor progress, and adjust care plans as necessary. This role requires a skilled nurse who can perform detailed assessments, identify potential health issues, and work closely with the healthcare team to provide high-quality care to our residents

Key Responsibilities:

Conduct detailed and accurate initial and ongoing resident health assessments, identifying medical, physical, and emotional needs.

Monitor and document changes in residents' conditions, reporting findings to the medical team.

Update and review care plans based on assessment results to ensure optimal care for each resident.

Administer medications, treatments, and other nursing interventions as prescribed by physicians.

Collaborate with physicians, care staff, and families to develop personalized care plans for each resident.

Provide health education to residents and their families regarding conditions, treatments, and self-care.

Ensure that nursing assessments and other documentation comply with regulatory standards and facility policies.

Respond to emergency situations promptly and provide critical care when necessary.

Participate in care team meetings to discuss care updates and changes in residents’ health status.

Qualifications:

Current and valid Registered Nurse (RN) license in NY.

Previous experience in assisted living, long-term care, or geriatric nursing preferred.

Experience in conducting resident health assessments and developing care plans.

Experience in the HCS UAS System

Current BLS/CPR certification.

Strong clinical assessment skills and attention to detail.

Excellent communication skills, with the ability to effectively collaborate with residents, families, and the healthcare team.
