Duly Health and Care **Lombard** is seeking an **APP** who is committed to empowering health and elevating care for our patients while also pursuing innovative solutions with their care team that move healthcare forward. This role includes a combination of outpatient clinic visits and dialysis rounding, providing comprehensive care to patients with chronic kidney disease and related conditions.**Key Responsibilities:*** Provide high-quality, evidence-based care to Urology/Uro-Oncology patients in both clinic settings.* Conduct thorough patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians.* Perform dialysis rounding, including evaluating patients, managing medications, and coordinating care with dialysis center staff.* Document all patient encounters and care plans accurately and efficiently in epic.* Communicate clearly with patients, families, clinic staff, and interdisciplinary teams.* Support nephrology providers by assisting with patient follow-up, education, and chronic disease management.* Participate in quality initiatives, care coordination, and process improvement efforts.**Opportunity Highlights*** Practice at the top of your license, forming trusted relationships and collaborating with leading physicians* Strong internal referral network and collaborative provider partners across **Urology/Uro-Oncology** and more than 1,600 Duly providers* Competitive salary, financial bonuses, CME funds, and 401K match* Live and work in the vibrant suburbs of Chicago with excellent schools and convenient access to some of the nation's leading healthcare facilities**Compensation & Benefits** The anticipated base compensation range for this position is **100,000-140,000**, depending upon the applicant’s qualifications and experience.
