Our Hospice Care team is seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes. Schedule We have multiple schedule variations available for this role. Saturday & Sunday 8 hour days Friday, Saturday & Sunday or Saturday, Sunday & Monday 8 hour days Please note all schedules will include alternating weekend on call expectations Your Responsibilities: As a member of our dedicated team, you'll: Provide compassionate end-of-life care, ensuring the physical and emotional well‑being of patients and their families. Deliver expert pain and symptom management, offering comfort and support during challenging times. Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients. Minimum of two years clinical med‑surg and acute care nursing experience required. Minimum of one year of Hospice experience required. Home care experience and/or previous case management experience preferred. Valid VT driver's license and reliable vehicle with automobile insurance is essential for home visits.
