Physical Therapist (PT) - Home Health

Therapist PT Home Health

Day shift, 5 days a week, 8-hour shifts from 08:00 to 16:30.

Contract length: variable; maximum 7 days of requested time off.

Requirements

• 1 year of experience; first timers accepted.
• Current BLS certification required.
• Valid state license or Compact PT license acceptable.
• Experience with Home Care Home Base (HCHB) preferred; online training available for $250 if needed.
• Familiarity with patient population including adults to geriatrics.
• Experience with joint replacements, COPD, CHF, and T2DM.

Responsibilities

• Provide physical therapy services in home health settings.
• Manage a daily patient census averaging 175 visits.
• Ensure patient care is effective and efficient, minimizing travel distance when possible.
• Adhere to compliance and documentation standards.
• Participate in clinical reviews to ensure fit within guidelines.

Compliance / Notes

Full compliance is required to start on time; delays may result in a push back of 2 weeks. Additional orientation hours may be required based on contract length.
