The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.
We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Oregon located in Downtown Portland, OR. This is a full-time, on-site position working from Monday-Friday from 8:00AM - 4:30 PM schedule with some holidays required.
Key Responsibilities
- Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
- Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
- Assist patients with understanding their medical care, insurance coverage, and available financial resources.
- Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
- Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
- Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients' needs and help resolve any issues or barriers to care.
- Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.

Position Requirements
- Education: Highschool Diploma required. Bachelor's degree (preferred)
- Experience: Minimum of 2 years of experience in a clinical healthcare setting, including patient care coordination, patient navigation, or case management.
- Skills:
- Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
- Excellent communication, organizational, and problem-solving skills.
- Proficiency in using electronic medical records (EMR) and other healthcare software.
- Bilingual skills are a plus.
Compensation & Benefits
- Hourly Rate: $26.00 - $30.00 per hour (Final offers based on experience, skills, and qualifications).
