CARE COORDINATOR

$20.25 per hour

Care Coordinator (Pueblo, CO) Hourly rate: $20.25. Position located at Pueblo, CO 81001. Position Overview Provide care coordination for patients in the program, guiding them toward recovery and supporting their overall well‑being. Responsibilities Develop care plans and conduct patient needs assessments. Facilitate Life Skills groups in accordance with company policies. Serve as liaison, establishing networks and collaborating with hospitals, community health centers, and state and human service agencies regarding patient services and needs. Document admissions, care management needs, discharge, and discharge planning. Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake. Organize patient bed dates, transition levels of care, and maintain communication with Medicaid Placement Coordinators. Assist treatment team with patient intake, transfer, and discharge assessments. Collaborate with facility treatment teams (medical, psychiatric, SUD) to identify patients’ needs. Assist patients in meeting basic needs (e.g., clothing, medical, judicial, religious) using available resources. Qualifications Associate or bachelor’s degree in a behavioral health‑related field is preferred, or a related field. Obtain Certificate of Addiction Technician (CAT) certification within 18 months of hire. Obtain case management certification within 90 days of hire. Minimum of two years of experience in the substance use and mental health field. Good understanding of substance use, mental health, and treatment delivery systems. Advanced computer skills required. CPR and first‑aid certification (agency funds initial training and renewals). Ability to take vital signs, recognize possible medical problems, and refer to medical staff as needed. Valid driver’s license with good driving record and current, unofficial DMV driving record upon hire. Effective communication skills in a multidimensional treatment program. Physical Demands Must assist other employees in moving clients, if necessary. Work Environment Position located in a residential setting. Benefits Medical, dental, and vision insurance.
