Tiburcio Vasquez Health Center is a mission-driven Federally Qualified Health Center (FQHC) serving the East Bay through ten primary care clinics, four school-based health centers, and multiple specialty programs. With nearly 400 employees, an annual operating budget of approximately $60 million, and more than 30,000 patients served each year, TVHC provides comprehensive medical, dental, behavioral health, WIC, youth health, and community-based services to some of the region's most vulnerable populations.
Job Summary:
The Senior Medical Director of Clinical Services is a senior physician leader responsible for supporting the Chief Medical Officer (CMO) in the operational and clinical leadership of Tiburcio Vasquez Health Center’s (TVHC) medical services and care delivery systems. This position partners closely with the CMO, Chief Operations Officer (COO), Associate Medical Directors (AMDs), Associate Director of Clinical Services, and operational leaders to ensure high-quality, accessible, patient-centered, and efficient clinical care across TVHC sites and programs.
The Senior Medical Director of Clinical Services provides day-to-day clinical leadership focused on provider engagement, ambulatory clinical operations, clinical quality execution, team-based care, and implementation of evidence-based standards and workflows. The role serves as a key liaison between frontline providers and executive leadership.
This position maintains an active clinical practice and provides direct patient care approximately 50% of the time.
The Senior Medical Director of Clinical Services reports directly to the Chief Medical Officer and may supervise Associate Medical Directors, Associate Director of Clinical Services and other clinical leaders as assigned.
Responsibilities:
Clinical Leadership & Provider Operations
- Supports the CMO in oversight of clinical operations across all health center sites.
- Provides physician leadership for ambulatory care delivery, promoting high-quality, evidence-based, patient-centered care.
- Partners with operations leadership to improve provider workflows, clinic efficiency, continuity, and patient access.
- Assists in reducing unnecessary variation in care through standardization of workflows, clinical protocols, and best practices.
- Collaborates with Associate Medical Directors, Site Managers, Nursing Leadership, and operational leaders to address clinical and operational issues.
- Supports implementation and optimization of team-based care models.
- Provides leadership related to provider schedule optimization, panel management, visit utilization, and template effectiveness.
- Participates in clinical operational planning and implementation of innovative care delivery models, including telehealth, co-visits, group visits, integrated care, and mobile care services.
Provider Leadership, Development & Engagement
- Serves as a clinical leader, mentor, and coach to providers across the organization.
- Supports provider onboarding, orientation, mentorship, and professional development.
- Participates in provider recruitment, interviewing, retention, and succession planning in collaboration with HR, Talent Acquisition, and operational leadership.
- Assists in addressing provider performance, professionalism, clinical documentation, peer review, and corrective action processes as appropriate.
- Facilitates provider meetings and promotes effective communication between providers and leadership.
- Helps foster a culture of collaboration, accountability, learning, service excellence, and continuous improvement.
- May supervise Associate Medical Directors and other clinical leaders as assigned.
Quality Improvement & Clinical Performance
- Supports implementation of TVHC’s quality improvement and population health initiatives in partnership with the CMO, the Senior Medical Director of Quality, Population Health & Clinical Informatics, Operations, and other leaders.
- Engages providers in achieving organizational quality, access, utilization, and patient experience goals.
- Uses clinical and operational data to identify opportunities for improvement and support provider performance.
- Assists in development and implementation of workflows to improve preventive care, chronic disease management, risk adjustment, and value-based care performance.
- Supports clinical documentation improvement initiatives and evidence-based practice standards.
- Participates in quality committees, peer review activities, and clinical performance improvement initiatives.
Integrated & Team-Based Care
- Supports integration of primary care, behavioral health, care coordination, Enhanced Care Management (ECM), Street Health, and enabling services.
- Collaborates with interdisciplinary teams to improve care coordination and whole-person care delivery.
- Promotes collaborative relationships between providers, nursing, behavioral health, outreach, care management, and operational teams.
- Assists in identifying and addressing barriers to access, continuity, and patient engagement.
Compliance, Risk Management & Clinical Standards
- Supports compliance with HRSA, HIPAA, CLIA, Joint Commission/accreditation standards, and all applicable federal, state, and local regulations.
- Participates in peer review, clinical audits, incident review, and risk management activities.
- Supports credentialing and privileging processes as assigned.
- Promotes adherence to clinical policies, procedures, and evidence-based standards of care.
Administrative Leadership
- Participates in organizational leadership meetings and clinical leadership committees as assigned.
- Collaborates with the CMO and operational leaders on strategic initiatives related to clinical operations and care delivery.
- Supports implementation of organizational priorities and change management initiatives.
- Performs other related duties as assigned.
Direct Patient Care
- Provides direct patient care approximately 50% of the time within area of specialty and scope of practice.
- Maintains high standards of clinical excellence, professionalism, patient experience, and documentation.
- Models patient-centered, culturally responsive, trauma-informed care.

Qualifications:
Education & Licensure
- Medical Doctor (MD or DO) degree from an accredited institution.
- Board certification in a primary care specialty required (Family Medicine, Internal Medicine, Pediatrics, or Medicine-Pediatrics preferred).
- Current unrestricted California medical license.
- DEA registration and current BLS/CPR certification required.
- MPH, MBA, MHA, or other relevant advanced leadership degree, preferred.
Experience
- Minimum 5 years of clinical practice experience in ambulatory care.
- Minimum 3–5 years of physician leadership experience in a community health center, FQHC, safety-net, or integrated care setting preferred.
- Experience supervising or mentoring providers strongly preferred.
- Experience with quality improvement, population health, value-based care, and team-based care models preferred.
- Experience working in culturally and linguistically diverse communities preferred.
- Familiarity with FQHC regulatory and operational environments preferred.

Salary Range: $259,305.25-$350,824.75 annually, depending on qualifications and experience.
